How to Prepare for New Medi-Cal Rx Requirements

California pharmacy claims requirements are changing.

Starting fall 2026, Medi-Cal Rx will require ICD-10-CM diagnosis codes on all pharmacy claims, including refills. Claims submitted without a diagnosis code will be denied and must be resubmitted.

Here’s what your pharmacy needs to do to stay compliant:

• Start submitting ICD-10-CM diagnosis codes on claims now*
• Verify diagnosis codes match prescriber documentation
• Contact prescribers if diagnosis codes are missing

*Medi-Call Rx accepts up to five ICD-10-CM diagnosis codes per pharmacy claim.
